After years of exaggerated lip filler and razor-sharp jawlines dominating beauty trends, the pendulum has been swinging back to a more natural look.
Celebrities including Courteney Cox, Anna Faris, Melanie Griffith, Kristin Davis, Olivia Culpo and even Simon Cowell have spoken openly about discontinuing or dissolving fillers, and we're seeing the same preference for a more balanced, proportional aesthetic extending beyond the bounds of Hollywood.
With this change, patients are increasingly asking questions about their options: Do dermal fillers dissolve on their own? Can fillers be removed if I don't like my results or don't want them anymore? Is it true that fillers might migrate or last longer than expected?
Symmetry Med Spa by Bengtson Center is an Allergan Top 100 and Black Diamond practice with an experienced team of Master Injectors and three board-certified plastic surgeons providing oversight. We stay close to the evolving science, and we ensure our patients do, too.
All dermal fillers are designed to be temporary. Your body's natural enzymes and lymphatic system gradually break down the filler material and your appearance returns to baseline. How long that process takes depends on the type of filler.
If you're looking for a "permanent filler," you may want to consider facial fat transfer. The procedure uses your own fat as the filler material, so there is no risk of allergic reaction, and the end result looks and feels completely natural. It is a longer-lasting solution than dermal fillers.
Different fillers dissolve at different speeds. Many of the most popular hyaluronic acid fillers, including products from the JUVÉDERM® and Restylane® families, last six to 12 months on average.
Hyaluronic acid fillers with a thicker, firmer consistency, such as JUVÉDERM® VOLUMA® and VOLUX®, tend to last longer. Many patients can enjoy their results for 18 to 24 months.
The longest-lasting volumizing injectables are biostimulatory fillers, including Sculptra®. Products in this category do not use hyaluronic acid. Instead, they use different active ingredients (poly-L-lactic acid in the case of Sculptra®) to add immediate volume while stimulating collagen production over time. Results can last up to two years or more.
Many, but not all, dermal fillers can be dissolved intentionally.
Hyaluronic acid-based fillers can be dissolved using an enzyme called hyaluronidase. This is one of the reasons they're so popular. They have a built-in safety net.
The reversible fillers include:
Biostimulatory fillers cannot be reversed because there is no dissolving agent available. Their effects must be left to fade gradually as the body metabolizes the product.
The non-reversible fillers include:
People choose to dissolve filler for various reasons, both personal and aesthetic. Some of the most common reasons for dissolving filler include:
You feel like something is "off" cosmetically. For example, if you have had lip injections, and you notice your lips look uneven or have an overfilled "duck lips" appearance, dissolving the filler can give you a clean slate.
The product has shifted from its original injection site to a nearby area. Filler migration is most common in high-motion, delicate areas like the lips, where constant muscle movement can push filler beyond the lip border, and the tear-troughs.
Visible or palpable bumps have formed under the skin. This can be caused by uneven distribution, product placed too superficially or the body's response to the filler material.
The Tyndall effect is a blueish hue that can occur when hyaluronic acid filler is placed too superficially, especially in an area where the skin is thin. It may be accompanied by visible swelling.
Less common but serious concerns such as vascular occlusion (when filler enters a blood vessel), infection or tissue changes that require medical intervention.
Sometimes it's simply a matter of preference. A patient may have loved their results initially but no longer feels the look reflects who they are now, or they may want a fresh start before taking a different approach to facial rejuvenation.
Dissolving filler with hyaluronidase is a relatively quick process. The injection appointment is similar to getting fillers, and the enzyme begins working immediately. Patients usually see results within 24 to 48 hours. Any fullness that remains after two weeks may indicate that additional hyaluronidase treatments are needed.
Hyaluronidase has a well-established safety profile and is used routinely both for elective filler reversal and emergency treatment of vascular occlusion. The most common potential side effects of dissolving filler are the same as receiving filler, namely pain, bleeding, bruising and swelling. Although it is rare, some patients may have an allergic reaction to the dissolving solution.
One consideration: hyaluronidase can temporarily affect your body's own naturally occurring hyaluronic acid in the treated area, so it should be used judiciously and only by experienced providers. We carry hyaluronidase at both Symmetry Med Spa locations and our Master Injectors are trained to administer it safely.
Most patients only need one treatment with hyaluronidase to dissolve their dermal fillers. Multiple visits (two to three sessions) may be necessary for patients with large amounts of filler or more durable types of filler.
Emerging research using imaging technologies such as MRI and ultrasound is showing that the visible effects and the biological presence of filler may operate on two different timelines.
In one study of 33 patients who received mid-face hyaluronic acid filler, researchers found the product was still detectable on imaging in 100 percent of subjects at two years. A 2024 meta-analysis of over 1,400 patients found that 50 to 86 percent of injected HA volume persisted at 24 months.
The evidence that dermal fillers may last longer than intended is not necessarily bad news, but it does impact how filler treatments should be planned.
At Symmetry Med Spa, we are committed to evidence-based practice. Our team continuously integrates the latest research into our protocols to ensure every treatment is founded on the most current science and results in a safe, beautiful outcome.
Here's what that looks like in practice:
We start with a thorough assessment. Every injectable consultation begins with an evaluation of your skin quality, facial structure, facial proportions and any existing filler history. Our VISIA® Complexion Analysis gives us an objective, data-driven view of your skin that goes well beyond what we can see on the surface.
We plan conservatively. "Less is more" is our motto. We strive for natural results always, and we increase volume gradually to avoid excessive or inappropriate filler accumulation. We may recommend longer intervals between filler sessions to avoid the "filler fatigue" or Facial Overfilled Syndrome (FOS) that comes from stacking layers upon layers of filler.
We choose products with intention. Different fillers are engineered for different depths, different areas and different longevity profiles. Our Master Injectors understand these nuances and match the right product to the right indication.
We're prepared for anything. Hyaluronidase is on-site and our Master Injectors are trained in vascular emergency protocols. Every treatment is backed by the oversight of our surgical team, Dr. Bradley Bengtson, Dr. David Alfonso and Dr. Narges Horriat, who are all board-certified plastic surgeons.
