Abdominoplasty (tummy tuck) is one of the world’s most popular cosmetic surgeries. According to the most recent data from the International Society of Aesthetic Plastic Surgery, it was the sixth most commonly performed plastic surgery procedure worldwide in 2024.
It’s even more popular here in the United States. The 2024 annual survey from the American Society of Plastic Surgeons (ASPS) ranked abdominoplasty the third most popular cosmetic procedure of the year, marking its fourth consecutive appearance in the top five.
But there is one aspect of the tummy tuck that isn’t so popular, and that’s the use of drains during recovery. Historically, tummy tuck patients have required one or two drains to remain in place for up to seven days after the procedure. These drains are valuable for preventing fluid buildup, but they can also be uncomfortable and inconvenient.
At the Bengtson Center for Aesthetics & Plastic Surgery in Grand Rapids, MI, board-certified plastic surgeon Dr. David Alfonso offers an alternative for patients seeking a different recovery experience: the drainless tummy tuck.
During abdominoplasty, the surgeon separates the abdominal skin from the tissue underneath and repositions it. That process creates a space between the layers. As the body heals, it produces various fluids, and those fluids can accumulate in the space left behind. Drains are placed to remove that fluid so it doesn’t build up and cause complications like seromas or infection.
Drains do their job well. They have been a standard part of tummy tuck recovery for decades. However, “effective” and “pleasant” are two different things. Managing drains means the patient must empty the bulbs regularly, measure and record the output, keep the drain sites clean and watch for signs of leaking or blockage.
Taking drains out of the equation removes a stressor that some patients feel is the most intimidating part of the process.
A drainless tummy tuck follows most of the same steps as a traditional abdominoplasty. The difference is how the procedure ends.
Dr. Alfonso uses rows of internal, absorbable sutures to close the space between tissue layers from the inside out. He often describes the process as similar to quilting: stitching the layers together so there is minimal movement and minimal open space where fluid could collect. The body gradually absorbs the sutures as healing progresses.

With a drainless tummy tuck, you never have to deal with cleaning and emptying drains, measuring and logging the output, or troubleshooting issues like clogging. There is no risk of a drain snagging on clothing or accidentally being pulled loose by a child or pet. There is simply one less thing to think about at a time when your full attention should be on rest and healing.
Tummy tuck patients recovering without drains tend to be mobile sooner. Without a cumbersome device attached to your body or worries that it might dislodge, it’s physically and psychologically more comfortable to move around. Walking after surgery supports healthy circulation and reduces the risk of blood clots.
A tube exiting the body creates a potential entry point for bacteria. Drain sites carry a small but real risk of infection, so it is important to follow good hygiene practices when managing the drains or touching the area. While no surgical procedure is risk-free, a drainless technique reduces the risk of infection by eliminating that entry point.
Tummy tuck patients often describe the small, practical annoyances as the hardest part of having drains. They affect what positions you can sleep in, when and how you can shower, what clothing you can wear and how comfortably you can move. Without drains, you have more freedom to live your life as you normally would.
Some people find the visual of fluid collecting in a bulb unsettling. Others worry about handling the drains or dread having the drains removed. A drainless tummy tuck eliminates these fears. For many patients, the peace of mind is invaluable.
Whether your tummy tuck involves drains or not, it is a significant surgery. Your body needs time and the right care to heal. The major recovery milestones are the same for a traditional tummy tuck and a no-drain tummy tuck:
First few days: prioritize rest
Week one: return to light activities
Four to six weeks: resume heavy lifting and exercise
Three to six months: see your final results
What differs is the recovery experience. Drainless tummy tuck recovery is not faster, but it is more comfortable.
The drainless technique does not change how much pain you might feel from the surgery, but it does remove a source of physical discomfort during recovery.
At the Bengtson Center, we address surgical pain through other advanced methods, including Exparel® and ultrasound-guided TAP blocks that provide long-lasting pain relief for our abdominoplasty patients and reduce the need for narcotic medication.
Dr. Alfonso has found that, in his experience, most patients are candidates for the drainless tummy tuck technique. He uses this approach across the full range of abdominoplasty procedures he performs, including:
